Do you play every day? Only on weekends? How long can you stay up on a 'raiding' night? We generally play from 7-11pm server time (CST) Sun->Thur; are those times you can make?

What are some past guilds that you have been with? Why did you leave them?

How long have you played WoW? What raiding or guild/raid-leading experience do you have? What are your best/favorite types of games that you excel at? Give us a general profile of your gaming experience and how good you are at games in general.

Do you have any real-life family or friends in Evo? If so, who?

Do you know anyone in evolution purely in-game (not in real-life), such as by questing together, happenstance, etc?

What alts do you have, and how often do you play them? If they are not on Stormreaver, please specify that.

Please list email/aim/icq/etc or other ways of contacting you out of game.

Deadly Boss Mods and threat addons (omen, skada) are required to raid; what other addons do you use? We don't need a full list, just some of the most important ones to your raiding. (Xperl? Clique? Grid? Decursive? Etc.)

Do you have Ventrilo and if so, a working microphone? We require vent for raiding, though only the ability to listen.

Do you have a Blizzard Authenticator? We require authenticators, so if you don't have one, when do you expect to get one?

We'd love to know who you heard about us from, or where you might have seen us around. Was it a forum post? A recommendation from a friend? Did you see us in game? We appreciate any feedback you can give us!
